MEDICAL FORMS – REQUIRED FOR PARTICIPATION
All campers must have approved medical documentation on file prior to arrival. Campers will not be permitted to check in without completed medical paperwork.
Medical Form Requirements
· Physician-signed medical clearance form
· Parent/guardian-signed waiver
· School physicals are acceptable only if they specifically state that the athlete is cleared for unrestricted athletic participation and include a physician signature

CAMP STORE & CONCESSIONS
Camp apparel, Penn State merchandise, pizza, and concession items will be available throughout camp.
